About Immigration Medical Exams

A medical examination is part of the application process for obtaining entry and/or residency to the United States for individuals of all ages. An adult examination (15 years or older) typically consists of the following information:

~ Physical examination

~ Vaccination records

~ USCIS required blood tests

The Day of Your Examination

To better serve you, please bring the following items with you on the day of your examination:

~ Government-issued photo ID (drivers license or passport)

~ Report of Medical History and Vaccination Record

~ Form I-693 located at uscis.gov website
